PROVISIONAL OPERATING LICENSE NO. DPR-45 IE INSPECTION REPORT NO. 50-409/79-17

Dear Mr. Keppler:

In response to your letter (Reference 1) which forwarded a notice of deficiencies as a result of the subject inspection, we are sub-mitting the following information for your consideration:

ENFORCEMENT ~ ITEM Deficiency:

1.

Technical Specification (TS) 3.5.1.5 requires a quorum of the Operatione Revieu Committee (ORC) ehalt consist of a chairman and six (e) membere including alternates.

Contrary to the above, on May 2, 1979, the ORC met tn conduct TS required businese with only four members in attendance.

DPC RESPONSE:

The meeting referenced in the inspection report was held after normal working hours during a refueling outage.

It was held for the sole purpose of reviewing six temporary changes to, procedures withia the specified ten-day review period prescribed in Technical Specifications.

The action.of holding a meeting of a dub-committee of the Operations Review Cofamittee for review of routine matters 0 lG L.

It should be noted that the number of participants in the sub-committee was six individuals including the Chairman.

The use of sub-committees, although operationally convenient, has been suspended for e.eetings convened to discuss topics set forth in Technical Specifications.

ENFORCEMENT ITEM Deficiency:

Technical Specification 3.8.2 requires each procedure and administrative policy of 3.8.1 and changea thereto, shall be revieued by the ORC and approved by the Plant Superin-ter. dent prior to implementation and every tuo years there-after.

Contrary to the above, Figure 2.4.1 of procedure 2,4, " Pre-dicted Critical Control Rod Position" used for ctartupe in May, June and July 1979, had not been reviewed or approved."

DPC RESPONSE:

Procedure 2. 4 " Predicted Critical Rod Position" is updated by the Reactor Engineer after initial startup testing for each cycle.

For the startups conducted in May, June, July 1979, the Reactor Engineer developed revised critical prediction curves which were presented to the Operations Department with related discussion.

The changes, although explained and discussed, were not documented as a temporary change to the Operating Manual Figure 2.4.1.

At times the Reactor Engineer was in communication with the Shift Supervisor during startup to discuss the applicability of the changes.

The Reactor Engineer has the responsibility for developing this information and insures that it is understood by the Operations Department personnel involved in startup.

Figure 2.4.1, "29 Rod Critical Bank Position" was revieNed by the Operations Review Committee and approved by the Plant S,uperintendent on July 31, 1979.
