e Appendix A gjg NOTICE OF VIOLATION Toledo Edison Company Docket No. 50-346 Based on the inspection conducted on May 29-31, June 1, 4-8, 11-22, 25-30, and July 2-8, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. These two items are infractions.
1.
Criterion V of Appendix B of 10 CFR Part 50 requires that " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings." Paragraph 17.2.6 of the FSAR states that
. Procedures have been developed to ensure that... oaly current.
documents are being used."
Procedure QAP-2060,
" Document Control," requires in Section 7.1.4 that ".
. changes be incorporated into documents associated with the change in a timely manner."
Contrary to the above:
a.
Drawing M-033 was never revised to reflect the fact that valve DH 100A was either never installed or was removed after installation.
b.
SP 1104.04.9 checklist was never revised to reflect the fact that there was no valve DH 100A.
c.
Drawings E547 and E49B, Sheet 25, were never revised to reflect the revised installation per FCT 77-155 on June 21, 1978, of relay TDR-2.
2.
Technical Specification 6.8.1 requires written procedures to be established, implemented and maintained.
Procedure AD 1810.01.1, Section 3.1.1 states in part that " combustible material shall not be stored in areas containing safety related equipment." Switchgear Room B contains breakers whica supply safety related equipment.
Contrary to the above on July 7, 1979 the inspector observed flam-mable materials in Switchgear Room B.
