- FINAL REPORT CONTROL ROOM PANEL POTENT! ALLY REPORTABLE DEFICIENe.Y No. 78/04 I. Description of the Deficiency:

General Electric - CR2940 control switch assembly requires a locking ring which secures the switch in place on the benchboard face. An error in canufacturing documentation utilized to manufacture control room panels was responsibic for the General Elect ric-CR2940 cont rol switch to be installed without the locking ring. /ei a result, the ent ire swit ch body nay rotate rather than the operator portion of the switch, thereby prevent ing the switch function.

II. Safety Implications:

General Electric - CR2940 cont rol switches are utilized for safety related manual cont rol functions. The omission of the locking ring from the switches could delay manual operation during an analyzed plant event such t hat technical specification limits could be exceeded.

Although t his deficiency is legally reportable, a significant safety hazard does not extst. The only panels which General Electric has already shipped that use CR2'340 handswit ches in safety related applicat ions are lill3-P870, 1H13-P870, and lH13-P872. The only safety function on these panels requiring operator act ion is init iation of hydrogen recombiners following an accident. However, the re-conbiners are not required until several days af ter an accident, and the plant staff would have adequate t ime to mal <c associated control switches operabic before safety limits were exceeded.

III. Corrective Action Taken:

The nanuf act uring documentation has been revised to assure installation with t he locking rings on control panels for Unit 2. The control panels far Unit I have already been supplied to the site, locking rings, installation tools and correct installat ion instructions have been provided for on-site installation.

General Elect ric's Cont rol and Inst rumentat ion Department, the designer and manuf act urer of t hese syst ens has inplemented changes to its Quality Assurance Program to prevent similar occurences. Addit ional details of the corrective action taken will be documented in General Electric Field Deviation Instruction WAEG.
