APPENDIX A NOTICE OF VIOLATION Baltimore Gas and Electric Company Docket No. 50-317 Based on the results of an NRC inspection conducted on April 16-20, 1979, it appears that one of your activities was not conducted in full compliance with NRC regulations and the conditions of your NRC Facility License No.
DPR-53 as indicated below.
This item is a deficiency.
Technical Specification Section 6.8.1 requires that " written procedures shall be established, implenented and maintained...."
Calvert Cliffs Instruction - 3003, Calvert Cliffs Operating Manual, Change 7, January 18, 1979, Part IX, states in part "The Locked Valve Deviation List...shall be used whenever a valve designated as ' locked' on the Valve Lineup List is placed in other than its designated position.
When the deviation is initiated, it shall be logged and initialed in the 'out of normal position' column."
Contrary to the ab'.ve the inspector noted on April 18, 1979, that the number 13 Charging Ptanp Suction and Discharge Valves, CVC-176 and CVC-178, were shut and tagged shut but were not on the Locked Valve Deviation List as being in other than the Designated Position. Vailvas CVC-176 and CVC-178 are designated " locked open" on the Valve Lineup List. These valves were being controlled by the Calvert Cliffs Safety Tagging Procedure.
The inspector verified prior to the completion of the inspection that NPE-0 Standing Order No. 79-02, had been issued and made available to all plant operators.
NPE-0 Standing Order No. 79-02 states that the Locked Valve Deviation List shall be used at all times including those occasions when the valves are under tagout or surveillance procedure control.
The inspector also verified prior to the completion of the inspection that valves CVC-176 and CVC-178 were returned to their designated positions.
Due to the above corrective actions no response to this item is required.
