Final Report For Reportable Deficiency No. 79/09 As Defined in 10 CFR 50.55(e)
I.
Description of Deff.ciency Raw material delivered by U. S. Steel Corporation to our supplier Tube Turns Division, Chemetron Corporation, was utilized to manufacture elbaw fittings wh!-h were found deficient.
These deficient elbow fittings were received at our construction site as material conforming with ASME SA 234, Grade UPB and identifed by Heat Numbcr W6719. The deficiency, due to a marking error of the raw material, resulted in material with a high carbon content being identified as material with a low carbon content.
In the absence of appropriate chemical and mechanical propertfes for the received elbows, the actual carbon content is indeterminate.
II.
Safety implications Carbon percentages above the limits permitted by the WPB Grade could conceivably promote cracking in the heat affected zone during welding of the elbows. Such cracking may not be evident when examined by non-destructive test.
This cracking may combine with certain severe service conditions and result in a loss of structural integrity at the weldment.
III. Corrective !.ction Taken Elbow fittings, installed and in storage, will be removed and shipped a
back to the supplier. Details of our corrective actions will be suzmarized in Bechtel Management Corrective Action Report, GGNS-49 and will be available for your review at the GGNS construction site.
