Appendix A SEP 12 619 NOTICE OF VIOLATION Atec Associates, Inc.
License No. 13-17732-01 Based on the inspection conducted on August 17, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Items 1 through 6 are infractions, and item 7 is a deficiency.
1.
License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations and procedurcs contained in application dated August 31, 1977, and letter dated March 1, 1978.
Contrary to the above-referenced application, it was learned through statements of licensee representatives that the individual designated as your radiation safety officer was changed and you failed to amend your license to reflect this change.
Specifically, your facility used byproduct material without an authorized radiation safety officer from August 1978, to date.

License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations and procedures contained in application dated August 31, 1977, and letter dated March 1, 1978.
Contrary to the above-referenced application, it was learned through statements of licensee representatives and the absence of records, that the requirement for quarterly calibration of survey instruments is not being met.
Specifically, your records indicated the last calibration was on August 11, 1977.

License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations and procedures contsined in application dated August 31, 1977, and letter dated March 1, 1978.
Cor' try to Addendum Items No. 4.0 and No. 4.1 of the above refer-ens.. application, which states no person is to use this equipment unless at all times the user is in possession of a film badge dosimeter.
It was determined by statements of licensee represen-tatives and the absence of records that this requirement is not being met.
Specifically, your records indicate Indianapolis employees wore film badges for only two months in 1978 and one month in 1979.

License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations and procedures contained in application oated August 31, 1977, and letter dated March 1, 1978.
Contrary to Addendum Items No. 5.1 and 5.2 of the above referenced application, which states "A quarterly physical inventory will be conducted to account for all sealed sources. The record shall be maintained for inspection." It was learned through st.tements of licensee representatives and the absence of records that this requirement is not being met.
Specifically, your records indicated the only quarterly inventory was made on June 30, 1979.

License Condition No. 13. A(1) requires that you test your sealed sources containing byproduct material for leakage and/or contami-nation at intervals not to exceed six months.
Coatrary to this requirement, as of the date of the inspection, you had not tested your scaled source serial No. 4474 for contamination or laakage since February 1978, an interval of more than six months.
6.
License Condition No. 12 requires that licensed material be used by, or under the supervision and in the physical presence of one of three individuals.
Contrary tc the move requirement, it was determined by statements of licensee represu.atives that licensed material was used by employees without the supervision and physical presence of an authorized indicidual.
Specifically, on the day of the inspection, licensed material was used at two locations and only one individual was autho ; zed to use or super-vise the use of the licensed material.

License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated August 31, 1977, and letter dated Mrrch 1, 1978.

It was 6starmined by observation of the NRC inspector that this requirement is not Lcing met.
Specifically, the storage cabinet and entrance door were not posted with " Caution Radioactive Material" signs.
