Dear Mr. Cornell:
Enclosed is the " Analysis of Three Mile Island-2 Accident" (NSAC-1), which is our first formal report on the TMI-2 accident.
The report represents the combined efforts of 65 people selected from 30 different organizations for their experience and knowledge in design, operation, and analysis which are relevant to the accident.
Included in the staff which prepared the report are 16 individuals with plant operating experience in 10 different utility companies.
Comments received from the industry on a series of draft versions of the report have been helpful in this published version of the report.
We also wish to acknowledge the excellent cooperation by General Public Utilities and Metro-politan Edison in providing full access to plant data and supporting information.
Babcock & Wilcox and Burns & Roe also cooperated in providing specific information as request-ed.
A series of supplements to this report are in preparation.
These will include a " Matrix of Equipment and Systems Actions" and the appendices " Hydrogen Phenomena" and " Operator Actions During Initisl Transients" not included in this edition.
A supplement " Core Damage Assessment Analysis" is also in prepara-tion.
While some analysis on the circumstances and phenomena involved in the accident will continue for several more months, the major efforts of the Center are now turned to defining and evaluating remedies for the causal factors for such accidents, and the lessons learned which may have more generic application.
This continuing work is being done in close cocrdination with the six Owners' groups and the industry committees working on the TMI event.
