I & E Inspection Report 79-13 Gentlemen:
This letter is in response to your Notice of Violation (I & E Report 79-13) from your inspection of our facility during the period July 10-13, 1979.
The responses are numbered to correspond with those used in your Notice of Violation.
- 1. (a) All control room operators have been readvised of the need to observe condition of their equipment and to issue PTR's when the equirment becomes inoperable. -
Those personnel directly involved have received a ver-bal reprimand for this occurrence.
In addition, Results Department personnel now make daily, Ibnday to Friday, inspections of all control room recordirg devices, with special attention to inking and drive mechanisms.
We are now in compliance.
(b) We have determined that the failure to log the re-quired data resulted because the area was inaccess-ible due to high radiation.
To avoid failing to note reasons for missing data the 12-8 shif t supervisor has been assigned the responsibility of reviewing all logs and resolving discrepancies.

The maintenance personnel involved have been verbally reprimanded for their failure to record their actions properly.
In addition, a newly formed group (plant QC) will, as they gradually assume their duties, conduct inspections of on-going activities and the records thereof.
The consulting firms recommendations (see response to 79-12) are expected to result in more meaningful, easier to use, procedures.
