Appendix A NOTICE OF VIOLATION Illinois Power Company Docket No. 50-461 Based on the results of an NRC inspection conducted on June 4-7, 1979, it appears that certain of your activities were in noncompliance with NRC requirements as noted below.
Items I and 3 are infractions.. Item 2 is a deficiency.

10 CFR 50, Appendix B, Criterion V, states, in part, " Activities affecting quality shall be prescribed by documented instructions, procedures
.. and shall be accomplished in accordance with these instructions."
Paragraph 3.5 of Baldwin Associates Procedure 2.3 titled Receiving and Issuance states, " Hold Tag, Form JV-177 - Used to identify unacceptable items and material to be moved to an established control area pending resolution of a Nonconformance Report."
Contrary to the above, the RIII inspector identified and the licensee acknowledged several structural components with bent clip-angles were stored with acceptable items without being identified with JV-177 tags.

10 CFR 50, Appendix B, Criterion IX, states, in part, " Measures shall be established to assure that special processes, including
- welding,
. are controlled and accomplished by qualified personnel using qualified procedures in accordance with applicable codes, standards, specifications, criteria and other special requirements."
Paragraph A of Chapter 9 of the Clinton Power Station Quality Assurance Manual states, in part, " Interfaces exist between the designing organizations, the fabricators and the constructor.
Interfaces are controlled by the designing organization specifying the applicable special processes in design documents and subsequently reviewing the special process procedures submitted."
Contrary to the above, it was determined that Zack Incorporated Welding Procedure Specification C-B-QCP22, Revision 2, dated July 6, 1978, was reviewed and approved even though the procedure erroneously stated that it was qualified to Section 5, Part B, of the American Welding Society D1.1-77, whereas it should have been Sdetion 5, Part A.

10 CFR 50, Appendix B, Criterion VII, requires that, ".
measures shall include provisions
. for objective evidence of quality furnished by the contractor or subcontractor.
" and that,
" Documentary evidence that the material and equipment conforms to the procurement requirements shall be available.
. prior to.
use of such material.
Clinton Power Station, PSAR, Appendix C, requires the licensee to conform to Regulatory Guide 1.28 (ANSI N45.2).
ANSI N45.2, Section 8, states in part that, ".
. measures shall include provisions for objective evidence of quality furnished by the contractor.
and that, ".
. documentary evidence that items conform to procurement requirements shall be waitable.
. prior to.
use of such items."
Contrary to the above, measures for the control of purchased material, equipment and services do not include provisions to provide objective evidence of quality for purchased non-shrink grout material prior to its use.
Specifically, over 700 fifty-five pound bags of Masterflow 713 non-shrink grout have been issued for use in both safety and non-safety areas without, as a minimum, written certificates of conformance which identify the requirements met by the items.
