I & E Inspect.:.on Report 79-12 Gentlemen:
This letter is in response to your Notice of Violation from the inspection (I & E Report 79-12) conducted at our facility during the period June 12-15, 1979.
The valves not signed off as checked open were in the proper position and no action was required. To avoid further incidents of failure to sign off procedures, and to enable management to determine those personnel responsible for carrying out assignments of this nature, preoperational valve lineups are now assigned, on a system basis, to one individual.
In addition, a consulting firm is presently reviewing the station procedural organization to advise on methods of making procedures more meaningful and easier to use.
This response was due to be sent July 31, 1979.
Its lateness is due to an administrative deficiency which failed to account for vacation schedules and reroute items to compensate for such schedules. The responsible personnel have been admonished verbally, and steps taken to keep people informed so that recurrences do not occur.
We believe we are now in compliance.
