APPENDIX A NOTICE OF VIOLATION Long Island Lighting Company Docket No. 50-322 This refers to the inspection conducted by representatives of the Region I (Philadelphia) of fice at the Shoreham Nuclear Power Station, Shoreham, New York, of activities authorized by NRC License No. CPPR-95.

During this inspection, conducted on March 27-30 and April 2-4, 1979, the following apparent item of noncomoliance was identified.

10 CFR 50, Appendix B, Criterion IX, statet, in part, that:

" Measures shall be established to assure that... heat creating (is)... accomplished

...in accordance with applicable codes, standards, specifications...."

The Shoreham FSAR, Chapter 17, Paragraph 17.1.9A, states in part:

"The LILCO EQA program imposes on organizations performing special processes on safety related structures, syste=s, and components, during the construction phase, the requirement to develop and implement a system to assure the control of special processes...The program requires that organizations performing special procesaes on safety related equipment...do so to qualified and approved procedures...and in accor-dance with applicable codes, standards, ;pecificatictis. c.."

Applicable GE Specification No. 21A9227AK impescs ASMF. Code Section III, Class C requirements for thermal cutting ou the residual heat removal heat exchangers. ASME Section III invokes, as candatory, ASME Code Section VIII, Appendix R, which requires a minimum preheat et 1750F for materials with specified carbon content exceeding 0.3% and wall thickness exceeding 1-inch.

Contrary to the above, in October 1976, the Courter Company crafts, under direction of Stone and Webster, performed thermal cutting of attachment welds to remove pressure caps from nozzles N3 and N4 of residual heat removal heat exchangers No. 034A and No. 034B, without qualified and approved procedures and apparently without performing preheat required by the applicable specifications.
