Appendix A NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-254 Based on the inspection conducted on May 29-June 27, 1979, it appears that certain of your activities were in noncompliance with NRC require-ments, as noted below. This item is a deficiency.

Technical Specification 6.2.A requires that detailed written procedures shall be prepared, approved and adhered to.
Contrary to the above, on June 13, 1979, the NRC inspector observed that IRM Channel 17 was bypassed, and this condition had not been logged as required by QAP 300-8.
