This refers to your application dated June 19,1978 (134.03-Ser 215) requesting approval of your Quality Assurance (QA) program as meeting the QA program requirenents of 10 CFR-91.51.

In connection with our review, we need the infonnation identified in the enclosure to this letter.

Please submit seven copies of your response to the enclosed request for additional information wit in 30 days following receipt of this letter. Also, other than Enclosures A and B of your application, please delete all enclosures from your application.

While there must be supporting written procedures to implement your QA program, these shculd not be submitted as part of your QA program plan.

Request For Additional Information 1.

Provide a commitment that design ~and fabrication shall not be conducted under this QA p*ogram.

Part 8.3 indicates the Radiological Protection Officer is involved in all inspecticns and audits (except the N&T In-House Inspection),

while Part 2.2.3 indicates this individual has important responsi-bilities for the program. Although the staff agrees with the need for and usefulness of the audits described in Section 8, the staffs position is that an annual audit of the adequacy of the entire effort be performed by individuals who have no responsibilities fn the activity being audited. Please revise Part 8 and provide a statement that an annual audit is performed by an audit team whose members have no responsibilities in the activity being audited.

Provide a commitrent that the Radiological Protection Officer shall assure that all radioactive material shipping packages are designed and manufactured under a QA program approved by Nuclear Regulatory Comnission for all packages designed or fabricated after the effective date of the QA program. The requirement can be satisfied by receiving a certificate to-this effect from the manufacturer.
