Appendix A NOTICE OF VIOLATION Monsanto Research Corporation License No. SNM-567 Based on the inspection conducted on May 21-24, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Item 1 is an infraction and Item 2 is a deficiency.
1.
10 CFR 20.101(a) limits the dose to the hand of an individual in a restricted area to eighteen and three quarters rems per calendar quarter.
Contrary to this requirement, an individual working in your restricted area received a dose to the hand of 21.88 rem during the first calendar quarter of 1979.

10 CFR 20.103(b)(2) states whenever the intake of radioactive material in any period of seven consecutive days exceeds that which would result from inhalation of such material for 40 hours4.62963e-4 days <br />0.0111 hours <br />6.613757e-5 weeks <br />1.522e-5 months <br /> at the uniform concentration specified in Appendix B, Table 1, Column 1, then the licensee shall make such evaluations and take such actions as are necessary to assure against recurrence. The licensee shall maintain records of the occurrence, evaluation and actions in a form suitable for review.
Contrary to these requirements, the licensee failed to record the action taken to prevent recurrence of the January 18, 1979 and Janua ry 19, 1979 intake of an individual of Pu-238 equivalent to a total of 47.3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br /> at the uniform concentration specified in Appendix B, Table 1, Column 1.
