NR dTUDYOFTMIINCIDENT As requested this morning, Bernie Snyder talked with Lee Gossick and Edson Case to get their thoughts en scoping of the NRC Study.
Also attached as Enclosure 1 is a proposed brief Public Announcement wnich was prepared and cooroinated with Frank Ingram.
The following points should be considered:
1.
I$ has already initiated a study which will investigate the causes, course of events, and imediate ccnsecuences, and need for possible enforcement acticns.
IE has started interviewing persons involved so as to not loose information which has not Deen recorded.'
2.
The IE investigation, with both Headcuarters and Regional Office involvement, should not be hindered by any subsequent Comission requested study (hereafter, referred to as the TMI Study).
3.
In pursuit of its licensing responsibilities the NRC licensing organizatic-will probably need to undertake its own investigation in order to implement any technical specification changes and safety evaluaticns.*
(NRR has already initiated a Task Force to examine this accident.)
4.
The licensee will be conducting a study, which will be of value to the TMI Study.

The NRC already has an established mechanism fcr conducting the TMI Study.
One outcome of the Browns Ferry Fire was a provision for an Incident Investigation Review Comittee (IIRC).
The details of the IIRC, as given in NRC Manuai Chapter 0502, are attached in Enclosure 2.
Note that except as otherwise prescri::ed by the Ccmission, an investigation of incidents is made under the auspices of the EDO.
It is clear that the magnitude of the TMI incident dictates that the TMI Study be established by the Co, mission.

Sccce of TMI Studv--Concectual Framework The basic questicns to be answered would seem to include:
How did the accident occur?
Were there any early indications that a problem existed at TMI, response to which could have prevented the accident?
How can similar events be prevented in the future?
(. A more detailed list of aspects se;3ested to be covered in the TMI Study will be. given below. )
. The conduct of the study prcbably should:
Be under Commission direction; Study group primarily censisting of NRC individuals, perhaps 4-5 representing various re:evant disciplines; Chairmanship of study grcup perhaps by a we.ll-cualified individual outside NRC (per suggestion of Lee Gossick);
Outside consultants utilized as necessary; ACRS input obtained; Scope of review should cover period up to the time when the plant is placed in cold shutdcwn condition (i.e., should not extend study to cover clean-up and recovery operations);"
As a rough guess, a thorough study and recort would take a minimum of six mor.ths (Lee Gossick agrees with the estimate).
As a point of reference, the Browns Ferry Fire report took almost 11 months; and
- Since there will be press'ure frcm Congress, public, etc., to get a report out much sooner, you may decide to receive an interim report.
Alternatively, the IE report may suffice as an interim report.

- Review secuence of events, causes, and consequences from IE investigation; Specific plant and generic design and operating procedure impli-cations;
- Societal impacts; Quality assurance practices;
- Operator induced scenarios; NRC management of incident; Outside influences on NRC which affected management of incident; Federal / State coordination; Emergency planning; 3
Information release and reporting; Feedback of incident to other operating reactors.

I GRC REVIEW OF THE THREE MILE ISLA"D ACCICE"T The,1uclear Regulatory Ccamissicn intends to cor. duct a prompt and comprehensive inquiry into the Three Mile Island Accident.
The review vill be independent of investigations or reviews by other groups and will be directed to determining what actions the Commission should take, in light of the Three Mile Island accident, to assure that the Commission is meeting its statutory responsibility for providing adequate proteccion of the public health and safety.
There will be two distinct, but not entirely separable, phases of the Commission's inquiry:
The first phase would be conducted for the purpose of establishing the exact nature and chronology of the
. events leading up to the release from the Three Mile Island plant of hazardous radioactivity and the creation of a situation potentially dangerous to the general public; The second phase would be conducted for the purpose of establishing what actions were taken, and when, to assure protection of the public health and safety from the con-sequences of the accident.
The first phase will censist primarily of the testimony of those engineers and superviscrs directly responsible for the operatien of the TMI unit.
It may also invol te testimony of those responsible for the design, instal-latien, maintenance, inspectica of structures, systems or components involved in the accident.
Accurate and unambiguous technical findings

are required from this phase of the inquiry in order to determine whether to impose regulatory requirements --in hardware or procedures-- for avoiding such an accident in the future.
The second phase of the inquiry will be concerned with the plant /public interface and will include testimony by plant management; NRC staff; local, state and federal (non-NRC) officials who were involved in alerting the public to the emergency and taking steps to meet the emergency.
Following this inquiry the Commission plans to set up a task group to determine what should be learned from the TMI accident and used to improve reactor safety and make the recurrence of a similar accident highly unlikely.
Based on the task group's recommendations, the Commission will decide what regulatory actions, including rulemaking, shculd be taken.

Proposed Public Announcement NRC Chairman, Joseph Hendrie has announced that an NRC incuiry of the accident at the Three Mile Island (TMI) Nuclear Facility is planned.
The Commission's incuiry will be conducted by a special high level Task Force reporting directly to the Ccmmissioners.
This Task Force will investigate the events leading up to and during the accident which started on March 23, 1979 at the TMI plant, located near Harrisburc, Pennsylvania.
The Task Force will establish the exact nature and the causes of the accident, wnether any indications of an imcending prcblem were available before the accident, how similar events can be prevented in the future, and what the implications may be for other nuclear power plants.
Details regarding this NRC investigation will be announced.

PA has recommended that a detailed backgrcund statement on the TMI accident is nct necessary.)
