SUBJECT:
RECORD OF ACTIONS - THREE MILE ISLAND ACCIDENT The following is transmitted in response to Mr. L. V. Gossick's request of April 13, 1979 on the above subject.
On April 2, 3, and 4,1979, I discussed with Messrs. F. Rosa, C. Miller, M. Srinivasan and R. Fitzpatrick areas of concern to me that needed attention to assure that the ever important Reactor Coolant Pumps remain functional and the Containment integrity intact:
First, I pointed out the high likelihood of loss of offsite power from a number of conditions and particularly lightning.
Therefore, s
I suggested that if power distribution interconnections at the plant allowed the back feeding of power from the onsite diesels to the rear. tor coolant pump busses, such back feeding would not violate electric power transfer principles ar.J diesel paralleling may provide sufficient capacity fer one reactor coolant pump.
Second, I suggested that sensitive instrumentation of differential over current protection for reactor coolant pump motors could be disabled to assure that low ground current differences do not unnecessarily trip the motors; as long as other less sensitive straight over current protection is provided and can be provided for real and significant over currents.
Third, I suggested that the electrical penetrations be evaluated to ascertain that, in the event of electrical faults inside containment, principally on reactor coolant pump motors or cable, the thermal stresses on the penetration will not cause breach of containment.
Fourth, reoarding qualification of reactor coolant pump motors to radiation environment I directed their attention to some of my files on Allis Chalmers motors and left my motor files with R. Fitzpatrick.
