Three Mile Island Nuclear Station Unit 2 Docket No. 50-320 Service Building Concrete
Dear Mr. Kruesi:
You were notified on June 14, 1973 of a situation which could be considered reportable in accordance with 10 CFR 50.55(e)(1)(1), " Conditions of Construction Permits". This situation relates to concrete place =ents for Three Mile Island Unit 2, Service Building.
Descriction of Incident Between February 23, 1973 and April 12, 1973, seven pours of -
crete (412 yards) were made ar.d treated as Class II concrete in lieu of Class I in the Service 22ilding. These seven placements consiscad of on interior wall, one slab, four curbs and stairaells. During this period, Quality Control personnel did not perform pre-placement or pour inspections; however, in keeping with accepted construction practices, batch plant forms, truck discharge nonitoring records, and concrete cylinder strength tests were executed and are available.
The incident occurred when the QA/QC persor.nel used the PSAR to determine QA/QC requirements for concrete placements in lieu of the Burns and Roe superstructures specification. The PSAR does not identify the Service Building as a Class I structure.
Corrective Action Deviation Report No. 0179 dated June 14, 1973, was issued to docu-ment the incident and to effect corrective acticn. The Deviation Review Board reviewed the existing pre-place =ent and pour inspection check sheets and accepted

In addition, both the construc-tion forces and Quality Control persennel have '::cen =ade aiare that QC cover-age is required on the Service Building. The Architect / Engineer is preparing a source docu=ent to be used by QA/QC persennel to identify all cc=ponents and structures requiring QC ccverage for future use.
