Dear Sir:
~hree Mile Island Nuclear Station Unit 2 (~1C-2)
License No. C??R-66 Docket No. f0-320 Energency Diesel Generaters On March 4, 1977, Mr. L. Narrow of ycur office was verbally notified of a s':uatice which Metropolitan Edisen Cc= pan 7 censidered te be reportable in accordanc.e vich the require =ents of ICCFR50.55 (e).
~his letter cens:1:utas the requ:. red thirty-day f ollow-up letter.
Descriction Fairbanks Merse Engine Divisic; of Col: Industries, the nanufacturer of the OC-2 Diesel Generators, advised us that, based on serrice reports en sinilar units in opera:1cn, sene cc=penents are not providing the intended service life.
Speci-fically:
1.
7er:1 cal Drive Eubs - It is possible that the hubs do no: =eet the nanu-facturer's app cpria:e material standard and should be replaced to prevent pessible crack' g failures.
2.
Fuel Header Cresscver Ccunection - A new, heavy duty, high alley pipe connec:ce and cla=p for each side of the engine should be provided to prevent cracking failure of the connec:crs.
3.
Ca= Shaf: and Ca= Follevers - Inproved ca= roller design d:h radiused edges, depth hardening and concentricity centrols shou 12 be installed to Ca= feliever fractures and/or c2= distress en the ca= shaft in preven:
order c achieve nor=al par: lifa.

n Analrsis of Safe:v *:clicarices Of the :hree cc=ponent problems described, cely the failed fuel header cross-over cennection failure caused cc=plete loss of diesel functices at another l
facility. The hub failure caused no loss of functices and the cart follever failu e caused slight loss of pcuer.
"'he =anufacturer has indicated that each inciden: occurred cely cece and that the probabili:7 cf addi:1ccal failures (had the ce=penents co: been corrected) is very Icv ccesidering that sc=e 25 diesels of the sa=e desig= have been in cperatice for 1 to 7 years.
I: the W d'<ely event that one diesel was ics: due :o a ce=ponen: failure at
~M'-2, the : -
- g 100:: redundant diesel veuld take over vital functicus.
For this reason, there veuld have been =o adverse effect ce the health er safety of the public or the plant staff.
Co nective Actice he folleving acticas are being planned Oc ec =ect these deficiencies:
1.
Fairbanks Morse vill perfer= a =lerescopic ex,-d atice of the vertical drive hubs. Those feu=d to be sub-standard vill be replaced.
2.
Fuel leader Crosscver Cc :ectices will be replaced by Fairbanks Morse with the new heavy duty, high alley pipe connec:c: and clamp en each side of each engine.
3.
New i=preved ca follevers vill be installed by Fairbanks Morse.
If inspectice shews evidence of ca= distress ce the c2= shaf:, 1: too vill be replaced.
4.
A fcur-hcur lead. test vill be perfor=ed ce the diesels af ter repairs are
=ade and a final inspectice vill be conducted.
All corrective acticus vill be ec=pleted prior ec fuel lead prese :17 scheduled for 10/15/77.
- spectice, repair and test documentatics -ill be available at
- he si:e for review.
Very truly ycurs, J
