NRC INVESTIGATION REPORT 4-2014-025

Dear Mr. Anderson:

This refers to the investigation completed on December 21, 2018, by the U.S. Nuclear Regulatory Commission (NRC) Office of Investigations (OI) at Arkansas Nuclear One. The investigation was conducted, in part, to determine whether two contract engineers employed at Arkansas Nuclear One willfully certified work on a record that was incomplete and inaccurate and whether one of the contractor engineers willfully caused Entergy Operations, Inc. to be in violation of Procedure EN-MA-119, Material Handling Program, associated with the stator drop event that occurred on March 31, 2013.

On June 23, 2014, the NRC issued two Yellow findings with associated violations to Entergy Operations, Inc. for the failure to follow procedures to ensure that a temporary lift assembly was adequately designed to support the projected load and to perform a 125 percent load test for the projected load (Agencywide Documents Access and Management System (ADAMS) Accession ML14174A832). On January 22, 2015, the NRC issued two Yellow findings with associated violations to Entergy Operations, Inc. for the failure to protect safety-related equipment from flooding which was recognized as a result of damage that occurred in the stator drop event (ADAMS Accession ML15023A076). On June 17, 2016, the NRC issued a Confirmatory Action Letter which included the corrective actions taken in response to the stator drop event (ADAMS Accession ML16169A193). The NRC conducted eight Confirmatory Action Letter follow-up inspections and closed the Confirmatory Action Letter on June 18, 2018 (ADAMS Accession ML18165A206) based on the NRC concluding Arkansas Nuclear One adequately addressed the associated corrective actions.

After considering the information developed during the investigation, the licensees completed corrective actions, and the quantity and rigor of the follow-up inspections that have been completed associated with the stator drop event, the NRC has determined that any enforcement action, as a result of its review of the subject OI report, was adequately addressed by previous enforcement actions and the Confirmatory Action Letter. Therefore, the NRC has decided not to pursue enforcement action that may have resulted from the subject OI report review.
