Initial Examination - Completion of Post-Examination Analysis and transmittal of final examination materials for ANO, Unit 2 (CE)
Dear Mr. Kirkland,
Contained in this submittal are the post-examination materials as required per NU REG 1021 Rev. 11 for the Arkansas Nuclear One Unit 2 Initial license examination administered during the week of April 22, 2019 and the written examination administered on May 2, 2019. The items that were given after the written examination are not included in this submittal. Also enclosed is a copy of the examination security agreement that has not been finalized due to the need to obtain post examination signatures.
We also request that all examination materials related to the ANO Unit 2 2019 NRC Operator License Examinations be withheld from public disclosure for a period of two and one half years from this date.

We also analyzed other missed questions based on candidate feedback following the examination and made changes to the exam database as needed/requested.

Unit 2 has tripped due to a LOCA.
SIAS has actuated.
RCS pressure is 1450 psia.
2LT-5636-1, RWT Channel A level transmitter fails low and has NOT been bypassed.
RWT level is 10%.

The RAS actuation requires that 2 out of 4 level transmitters to be less than 6%. The loss of power to a level transmitter causes it to read 0%.
Channel 1 is powered by 2RS-1 Channel 2 has an auctioneered power supply from 2RS-1 and 2RS-2 Channel 3 has an auctioneered power supply from 2RS-3 and 2RS-4 Channel 4 is powered by 2RS-4 A RAS initiation will close the Recirc isolations for the HPSI pumps. With 2LT-5636-1, RWT Channel A level transmitter failed low, a failure of 2RS-2 or 2RS-3 will not cause a RAS initiation but a failure of 2RS-4 will cause a RAS initiation. This is true because channel 2 and 3 have a backup auctioneered power supply.
Based on feedback from the applicants during the exam review, they were thinking transmitter failures only and did not remember or consider the auctioneered power supply logic thus selected the incorrect distractors.
After the review, the applicants agreed this was a valid question with the correct answer. ANO Unit 2 recommends that the answer should NOT be changed AND the question should NOT be deleted.

Based on feedback from the applicants during the exam review, they could not remember the exact wording of the step in the attachment and when practicing this attachment in the simulator had always defaulted to depressing both buttons on each panel as a conservative measure thus selected the incorrect distractors.
After the review, the applicants agreed this was a valid question with the correct answer. ANO Unit 2 recommends that the answer should NOT be changed AND the question should NOT be deleted.

Based on feedback from the applicants during the exam review, the six that missed this question could not remember any allowed procedural ALTERNATIVE ACTIONS that could be taken for the Main Turbine on the Turning Gear prior to drawing a vacuum thus picked the incorrect distractor D.
After the review, they all agreed this was a valid question with the correct answer.
ANO Unit 2 recommends that the answer should NOT be changed AND the question should NOT be deleted.

Based on feedback from the applicants during the exam review, the SROs stated that they misapplied or could not recall the correct time requirements in TS 3.5.2 and may have applied TS 3.0.3 time requirements instead thus missed the question.
After the review, they all agreed this was a valid question with the correct answer.
