Tennessee Valley Authority letter to NRC, "Annual Radioactive Effluent Release Report for 2018 Monitoring Period," dated April 22, 2019 In the referenced letter, Tennessee Valley Authority (TVA) provided the Annual Radioactive Effluent Release Report for the 2018 monitoring period at the Sequoyah Nuclear Plant (SQN). As part of Enclosure 2, Attachment 3, TVA provided an explanation for three instances of radiation monitors out-of-service for more than 30 days. Per the Offsite Dose Calculation Manual (ODCM), Section 5.2, an explanation is to be provided as to why non-functionality of liquid or gaseous effluent monitoring instrumentation was not corrected within the time specified of the ODCM.

Subsequent to the referenced submittal, TVA identified that a fourth instance of a SQN radiation monitor non-functional for greater than 30 days was omitted from the report. This omission has been entered into the TVA corrective action program.

The following is the explanation for the fourth instance.

On November 10, 2018 at 13:39, the Auxiliary Building Vent Monitor O-RM-090-0101B was removed from service to support 2B-B 6.9 kV shutdown board cleaning.

During restoration of the radiation monitor, unanticipated corrective maintenance was required. The radiation monitor was returned to service on December 20, 2018, at 15:33 after required repairs were completed. Compensatory actions were in place for the entirety of the non-conformance period.
