ISSUANCE OF SABIA, INC. AMENDED REGISTRATION CERTIFICATES NR-1195-D-103-S, NR-1195-D-104-S, AND NR-1195-D-105-S
Dear Mr. Pouliot:
This letter is in response to SABIA, Inc.s request dated March 26, 2019, requesting to amend Sealed Source and Device Registration Certificates Nos. NR-1195-D-103-S, NR-1195-D-104-S, and NR-1195-D-105-S to update the address for SABIA, Inc.s corporate headquarters. In addition, a typographical error was identified in one of the sealed source model numbers in NR-1195-D-103-S and NR-1195-D-105-S, and it was corrected. Copies of the amended registration certificate are enclosed.
Please be advice that you must manufacture and distribute the product in accordance with the statements and representations contained in your application, with enclosures thereto, and the information set out in your registration certificate. As a general rule, you must request and obtain an amendment to the certificate before you make changes or modifications to the information submitted to obtain the certificates.
Please note that a certificate of registration, by itself, does not convey authorization for possession or distribution of radioactive sources or devices. A U.S. Nuclear Regulatory Commission or Agreement State license is required for possession and distribution of radioactive materials, sources, or devices.
Please read over the registration certificate in its entirety and notify us immediately of any errors or omissions.
You are obligated to notify us promptly in writing if you decide to no longer manufacture, distribute, or offer service support for the product.
