Requisitions: OCHCO-19-0058; ZEROREQ-OCHCO-19-0047 The purpose of this contract modification is to:
(1) Exercise Option Year 4 thereby increasing the ceiling by $85,000 from $340,000 to
$425,000.00; and extending the period of performance from June 30, 2019, through June 30, 2020; (2) to incrementally fund the contract by $50,000.00 thereby increasing the obligated amount from $153,000 to $203,000.00; and (3) to change the primary COR from Frank Pavlechko to Andrea True and the Alternate COR from Andrea True to Douglas Simpkins.
