Dear Mr. Moseley:

A review of Surry Programs has been conducted as requested by your letter IE: II: NCM dated December 17, 1976 and IE Circular 76-

07. The following summarizes the results:

a.) Own plant Licensee Event Reports are placed in the Required Reading file which all personnel in the operations department read and initial. The Required Reading file is established by Administrative Procedures 29. LER's are also reviewed at each formal retraining session for licensed operators.

Other plant incidents are discussed and presented at monthly formal retraining sessions given to all licensed operators.

Operational errors or "near misses", i.e., events that are of less severity than reported incidents or License Events, are written up informally as "significant Operating Events" and presented to all Operations personnel at Weekly Shift Supervisor's Meetings and at Weekly Shift Safety Meetings.

b.) Annual operator performance evaluations are required to be performed on all operators by Shift Supervisors. Adherence to operating procedures, use of system checklists and implementation of component and system tagouts are major portions of operating personnel's duties, and thus their proper use is included in performance evaluations.

A bi-weekly audit of shift personnel compliance with Administrative Procedure 29 is conducted by a designated licensed operator, and discrepancies reported to the Operating and Assistant Operating Supervisors.

c.) Random Backshift or weekend visits are made by the Superintendent Station Operations, Operating Supervisor and the Assistant Operating Supervisor.

d.) The Station Quality Assurance Manual is the controlling document for all procedures, check lists, periodic tests, and equipment calibration. Personnel receive training with respect to their re-sponsibility in part of the general employee retraining program.

Only qualified personnel are assigned to accomplish tasks on safety related equipment. Each procedure, check list, test or calibration requires initials of the person performing the job. All results are reviewed for accuracy and completeness and signed by the cognizant supervisor. If the quality control personnel are required to participate in the procedure or test, it is annotated as a quality control hold point. They conduct periodic audits to ensure compliance with

/the Quality Assurance Manual.
