Boron Injection Tank Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Operation Reactor Power 17.5%

Plant Pressure 2235 Plant Tavg 552°F DESC.RIPTION OF OCCURRENCE:

On 12/25/76, the Boron Injection Tank was inoperable due to an inadvertent safety injection actuation which reduced the boron concentration below the minimum concentration allowed by Technical Specifications paragraph 3.5.4.1. The Shift Supervisor implemented EI-I.4.2 and plant conditions were returned to normal.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was an inadvertent safety injection actuation details of which will be contained in the 90-day special report submitted pursuant to Technical Specification 6.9.2 (a).

ANALYSIS OF OCCURRENCE:

The Boron Injection Tank was returned to an ope=abl~ status within the time allowed by Technical Specification L.C.O. paragraph 3.5.4.1. At no time was there any hazard to the general public or site personnel.

CORRECTIVE ACTION:

The Boron Injection Tank was returned to operable status in accordance

FAILURE DATA:

This is the third occurrence of this type (76-19/3L; 76-21/3L)

Component failure data is not applicable for this occurrence.

The Boron Injection Tank is a passive component common to both ECCS sub systems and, therefore, no redundant component exists. This is the third occurrence of this type (76-26/3L).
