Containment Pressure Protection, Channel II, PT-948C, Inoperable.

CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 3, Plant Pressure 2235 psig, Plant Temperature 547°F.

DESCRIPTION OF OCCURRENCE:

On 1/4/77, while performing the containment pressure protection channel functional check, Channel II, PT-948C, was found to be out of specifica-tions.

The trip point was found to be above the Technical Specifications limit as specified in Table 3.3.4.

The Shift Supervisor implemented Technical Specifications action statement 3.4.3.2(a).

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The dual comparator model 118 of the containment pressure protection Channel II (PT-948C) failed.

The comparator was replaced and restored to normal.

A calibration check was performed and the functional check completed.

ANALYSIS OF OCCURRENCE:

With the number of operable channels one less than the total number of channels, operation may proceed provided the operable channel is placed in the bypassed condition and the minimum channels operable requirement is demonstrated within one hour.

At no time was there any hazard to the general public or site personnel.

CORRECTIVE ACTION:

The dual comparator was replaced and a calibration check and functional check was performed.

The containment pressure protection Channel II was returned to normal service.

FAILURE DATA:

This is the first failure of this type.
