No. 12 Steam Generator Steam Flow Channel II Inoperable.
CONDITIONS PRIOR TO OCCURRENCE:

140 MWe DESCRIPTION OF OCCURRENCE:
On 1-20-77 No. 12 S/G Steam Flow Channel II was declared inoperable due to erroneous high steam flow signals.
Technical Specification Action Statement No. 14 for LCO 3.3.2.1 was implemented and No. 12 S/G Steam Flow Channel II was placed in the tripped position.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
A negative slope in the low pressure sensing line causes condensation to collect in the low point which~blocks the required steam flow to the condensate pot.
This blockage allows the steam in the sensing line to condense, lowering the pressure on the low pressure side of the steam flow transmitter causing a false high steam flow signal.
ANALYSIS OF OCCURRENCE:
Technical Specification Action Statement No. 14 for LCO 3.3.2.1 states that with the number of operable channels one less than the total number of channels, operation may continue until the performance of the next required channel functional test,- provided the inoperable channel
, is placed in the tripped condition within one hour.
No. 12 S/G Steam Flow Channel II was placed in the tripped condition within the one hour allowance, therefore the Technical Specifications were not violated.
At no time was there any danger to the health and safety of the general public or site personnel.

I Design chang~ ED-0047A was developed by the Engineering Dept.
This change modified the low pressure sensing line and the root valve orientation eliminating any possible condensate build-up.
After implementation of ED-0047A all new welds were examined by MT method.
New piping between the root valve and the transmitter was hydrostatically tested.
Piping between the root valve and the Main Steam Header was tested in-service at Hot Standby conditions.
FAILURE DATA:
This is the second failure of this type.
The Engineering.Dept.
is initiating a similar design change that should be accomplished on all Steam Flow Transmitters not already modified.
This change should be accomplished during the next scheduled outage.
