No. 12 Containment Fan Coil Unit Inoperable CONDITIONS PRIOR TO OCCURRENCE:
Operational Mode 1, Reactor Power 29.5%, Generator Output: 275 MWe DESCRIPTION OF OCCURRENCE:
At 1400 on 1/28/77,.No. 12 Containment Fan Coil Unit was declared in-operable due to an_ erroneous High Radiation Alarm on No. 12 Containment F.C.U. service water outlet.
Technical Specification Action Statement for L.C.O. 3.6.2.3 was initiated and No. 12 C.F.C.U. was taken out of service.
DESIGNATION:op AP~ARENT CAUSE OF OCCURRENCE:
The High Radiation Alarm on the service water outlet was caused by a high humidity condition in the area of the radiation monitors.
Noisture collected in the monitor housing resulting in high voltage spikes to ground which tripped the alarm.
Upon actuation of the alarm, the service water was automatically isolated.
ANALYSIS OF OCCURRENCE:
Technical Specification Action Statement for L.C.O. 3.6.2.3 states that with one group of containment cooling fans inoperable, restore the inoperable fan group to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in Hot Shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.
No. 12 Containment F.C.U. was declared operable within two hours, therefore, the Technical Specifica-tions were not violated.
At no time was there any danger to the health and safety of the general public or site personnel.
CORRECTIVE ACTION:
The radiation monitor was disassembled sufficiently to completely dry out the internal components.
It was reassembled and the case resealed.
The ventilation system for the 78' elevation north penetration was checked and a damper ~as found stuck in the shut position.
The damper was freed and properly positioned, which returned the 78' elevation envir0nmcnt to normal.
