This letter is in response to the mb microtec USA, Inc. request dated December 18, 2018, requesting to amend Sealed Source and Device Registration No. NR-0446-D-103-E to change the address of mb-microtec ag in Switzerland. A copy of the amended registration is enclosed.
Please be advised that you must manufacture and distribute the product in accordance with the statements and representations contained in your application, with enclosures thereto, and the information set out in your registration certificates.
