BWXT NUCLEAR OPERATIONS GROUP, INC., U.S. NUCLEAR REGULATORY COMMISSION INSPECTION REPORT 70-27/2018-412 AND NOTICE OF VIOLATION

Dear Mr. Burch:

This letter refers to the inspection conducted from October 29 - November 1, 2018, at your Mt. Athos facility located in Lynchburg, VA. The purpose of the inspection was to determine whether activities authorized by the license were conducted safely, and in accordance with the Nuclear Regulatory Commission (NRC) requirements. At the conclusion of the inspection, the findings were discussed with Mr. M. Hicks, Mr. A. Rander and other members of your staff on November 1, 2018.

This inspection examined activities conducted under your license as they relate to, fitness for duty program including human reliability and fitgue management, and a follow up on traditional enforcement activities. Within these areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations of activities in progress.

Based on the results of the inspection, the NRC has determined that two Severity Level (SL) IV violations of NRC requirements occurred. These violations were evaluated in accordance with the NRC Enforcement Policy. The first violation is cited in the enclosed Notice of Violation (Notice) and the circumstances surrounding it is described in detail in the subject inspection report. The violation is being cited in the Notice because it was idenfied by the NRC. The second violation was identified by the licensee. Upon discovery, the licensee took prompt and appropriate corrective actions including a commitment, within a reasonable time, additional corrective action to prevent recurrence. Therefore, the second violation is being treated as a Non-Cited Violation (NCV). The NCV is described in the subject inspection report. In accordance with Title 10 of the Code of Federal Regulations (10 CFR) 2.390, Public inspections, exemptions, requests for withholding, of the NRC's "Agency Rules of Practice and Procedure," a copy of this letter will be available electronically for public inspection in the NRC Public Document Room, or from the Publicly Available Records (PARS) component of NRC's Agencywide Documents Access and Management System (ADAMS); accessible from the NRC Web site at http://www.nrc.gov/reading-rm/adams.html (the Public Electronic Reading Room).

The enclosed inspection report contains Security-Related Information in accordance with 10 CFR 2.390(d)(1) and its disclosure to unauthorized individuals could present a security vulnerability. Therefore, the material in the enclosure will not be made available electronically for public inspection in the NRC Public Document Room or from the NRCs document system (ADAMS), accessible from the NRC Web site at http://www.nrc.gov/reading-rm/adams.html.
