Mr. Lewis has successfully completed adequate training through Nevada Technical Associates. Mr.

Lewis has been a trained radiation worker since September of 2011 when he joined Sensor Concepts and Applications (SCA). He has worked alongside the current Radiation Safety Officer (RSO) on many projects and has the complete confidence of the current RSO and was appointed to the position of Assistant Radiation Safety Officer (ARSO) in June of 2016 and is named on SCAs Maryland Radioactive Materials License as the current ARSO.

Mr. Lewis has trained radiation workers, general employees and others in radiation safety, has assisted in maintaining a radiation monitoring program and has been responsible for assuring license and regulatory compliance with the state of Maryland and the Commonwealth of Massachusetts, which SCA has had reciprocity agreements. He is also a certified shipper of DOT Class 7 materials.

Let me just say, looking over your draft, that you should submit documentation that your prospective RSO successfully completed RSO training:

I would like to see a certificate of completion, or documentation from an organization outside of SC&A, just for being above board on this. A record of completed AU training would be good as well.

The original license approved this NUREG RSO training. I wasnt a part of that review and I might have looked for something besides a Fixed-gauge qual, but that is what was agreed to and approved and I will honor that. I think it is adequate for the sealed SNM on this license. Give me some ballpark of when I should be seeing this LAR incoming. Like I said, you can send it to me and Ill get it docketed.

And please catch me up on what DNDO might want to do with this license. SC&A and Passport will come due in 21, so this is far enough out front for the organization to give this some thought and plan accordingly.

Ty, I will be retiring at the end of this year and I have written an amendment request to change the RSO on our license to my replacement. Should I send this to you via e-mail or to the Licensing Assistance Team in King of Prussia, PA?
