ABNORMAL OCCURRENCE REPORT Report No: BFAO-50-260/754W Report Date: January 31, 1975 Occurrence Date: January 23, 3.975 Facility: Browns Perry Nuclear Plant unit 2 Identification of Occurrence During an 'inspection 'of shock arrestors, the following four Bergen-Paterson arrestors weg'e found.,with an unacceptable 3.ov amount of oil present in the

One arrestor on the contro3. rod, drive pump suction line inside the drywell.

One arrestor on a residual heat removal. containment sprsy line outside the dryvell.

Two arrestors on the reactor core isolation cooling system turbine discharge line.

Conditions Prior to Occurrence The reactor was in the cold. shutdown condition during an outage.

Descri tion of Occurrence The deficiencies in the arrestor units were found during an inspection of a11.

Desi ation of A arent Cause of Occurrence The apparent cause of the low fluid level in the arrestors on the control rod drive and. residua1 heat removal lines was a small nick in'he sealing surface of the accumulator, piston seal. This seal contains an "0" ring inside a U-cup..

This "0" ring retards diametrical compression of the entire sea3. during insta1lation of the seal and. operation of the arrestor. The two arrestors on" the reactor core iso3.ation cooling system turbine discharge line had an unacceptable low amount of oi1 because all the po~ethane seals in the va1ve assemblies had not been replaced.. It appears that, during the rebuild program .

in which the'po~ethane seals vere to be replaced with ethylene propylene sea1s, an error was made which resulted. in 3.caving one,po~ethane seal in one arrestor and two polyurethane sea1s in the other. These po~ethane sea1s had. deteriorated and. a11owed. f3uid. to leak out..

An sis of Occurrence

. No damage to .systems, components, or structures was experienced; no personnel

. ~uries or exposures were involved.; no radioactive materia1s were released; and. there were no adverse effects to the public hea1th and, safety as a result of this occurrence.

<<2<< v Corrective Action The Bergen-Paterson arrestors which had. an unacceptable lov amount of'i1 were disassembled.,'nspected,, and. reassembled using the recommended. ethy3.ene prapy3.ene seals and. "0" rings. Care was taken to prevent inadvertent nicks on seal surfaces when rebuilding.
