NRC-HQ-84-15-G-0045 M0002 The purpose of the modification is to:
- 1. revise the period of performance; and
- 2. revise the Federal Financial Report (FFR) and Performance Progress Report (PPR) reporting requirements.
As a result of this modification:
- 1. the period of performance has been extended for one (1) year at no additional cost to the government. The period of performance has been extended from 9/29/18 to 9/29/19. The period of performance for the grant is 09/30/2015 through 09/29/2019.
- 2. the FFR and PPR reporting requirements found on page 16 of Attachment C - Standard Terms and Conditions have been revised as follows:
FEDERAL FINANCIAL REPORTS Federal Financial Reports (SF-425) are semi-annually, for the periods ending March 31 and September 30, or any portion thereof, regardless of the award date. Reports are due within 30 calendar days following the end of the reporting period and must be submitted through FedConnect. The SF-425 form and instructions are available at Grants.gov.
PERFORMANCE PROGRESS REPORTS The performance (technical) progress report indicated below is subject to 2 CFR §200.328.
Faculty Development Performance reports must be submitted semi-annually, for the periods ending March 31 and September 30, or any portion thereof, regardless of the award date. Reports are due within 30 days following the end of each reporting period and must be submitted through FedConnect.

Final Reports - The Recipient is required to submit final reports, both Financial (SF-425) and Performance within 90 days of the grant expiration. In addition to these reports, a final SF-428, Tangible property report, is also required, if applicable. The final PPR (for Scholarship, Fellowship, and Trade School and Community College Scholarship awards) must include the names of all students with up to date contact information (mailing address, telephone/cell phone, email address). The reports must be submitted through FedConnect.
