Gryglak contact Dr. Boote and asked to provide the following additional information in response to the 8/2/18 letter: 1) clarify the storage area for the radioactive materials used at the Spectrum Health Pennock Hospital 2) provide history of use, the radiation and contamination survey maps, survey results, equipment used, leak test results for the former Nuclear Medicine area at the Spectrum Health West Pavilion location.
- 3) Clarify the license number and location of use for Dr. Leffler's and Dr. McNamara's supervised training.
- 4) provide form 313a(aus) for Dr. Thieme listing the training and work experience for the requested use as the Board certification cannot be accepted per the NRC guidance (missing "AU Eligible" language above the ABR seal). 5) Correction of the radiation and contamination survey map information (description of survey points on the map did not match with the description of the points on the survey results sheet) to terminate a location of use at Spectrum Health Ludington Hospital).
