Please confirm in a signed and dated letter the correct address for the new area of use and release of a former Nuclear Medicine Department, Spectrum Health West Pavilion:

6105 Wilson Ave. SW, Wyoming, Michigan or 6105 Wilson Ave. SW, Grandville, Michigan.

1) Regarding termination of license no. 21-18667-01, Spectrum Health Pennock, please clarify the discrepancy between the sealed sources listed on the first page of attachment G and the inventory list, in letter dated August 2, 2018. 2) Regarding the new layout at the Spectrum Health West Pavilion located at 6105 Wilson Ave SW, Grandville, Michigan, please clarify the purpose of rooms 1027 and 1031, Echo Vascular.

Is there any imaging equipment, are these exam rooms, etc.? 3) Regarding the release of the former area of use at Spectrum Health West Pavilion and the information provided in your August 9, 2018 letter, please provide the following:

  • specific radioisotopes used
  • the background for the contamination wipes
  • Are the results provided in net dpm?
  • The action levels for ambient radiation surveys and the contamination surveys
  • Confirmation that radioactive waste has decayed to background or was moved to the new area of use
  • Confirmation that all radioactive material was removed from this area of use
  • Confirmation that the sealed sources were moved to the new area of use
  • Sealed source inventory and leak test results performed prior to May 15, 2018 4) Please confirm that you do not wish to add James M. Hecksel, D.O. to your license, license no. 21-16737-01.

Please provide information regarding questions 1, 2, and 3 in a signed and dated letter as soon as possible.

Please email your response to me (there is no need to mail the letter) so I can quickly process it. I will be back in the office on Wednesday, August 15. Please let me know if you have any questions.
