TERMINATION OF YOUR NRC RADIOACTIVE MATERIALS LICENSE NO. 21-16737-01, MEMORIAL MEDICAL CENTER OF WEST MICHIGAN D/B/A SPECTRUM HEALTH LUDINGTON HOSPITAL
Dear Dr. Boote:
In letters dated August 1, August 2, and August 9, 2018, and U.S. Nuclear Regulatory Commission (NRC) Form 314 dated July 31, 2018, Memorial Medical Center of West Michigan d/b/a Spectrum Health Ludington Hospital informed the NRC that it wished to terminate its NRC Radioactive Material License No. 21-16737-01.
We understand that radioactive material authorized for use on NRC License No. 21-16737-01 will be transferred to NRC License No. 21-00243-06 for Spectrum Health Hospitals and the facility will continue to operate under the Radiation Safety Program of Spectrum Health Hospitals.
Based on the information provided, NRC License No. 21-16737-01 is hereby terminated.
In accordance with Title 10 of the Code of Federal Regulations Section 2.390 of the NRC's "Rules of Practice," a copy of this letter and its enclosure will be available electronically for public inspection in the NRC Public Document Room or from ADAMS, accessible from the NRC Web site at http://www.nrc.gov/reading-rm/adams.html.
