Check, Chief', Reactor Safety Branch, DOR SINGLE FAILURE ON TURKEY POINT, UNHS 3 AND 4 In our review of the inservice testing program of valves for Turkey Point 4, we have identified a single failure in Turkey Point, Units 3 and 4, that could prevent the required ECCS flm from entering the vessel.

As shown in Drawing FPSL No. E-503185-7, Rev. 6, and RSAR Figure 6.2-1, Rev. 33, the tin Residual Heat Removal pumps normally inject ECCS flow into the three cold legs through valves 3-744A and 3-744B (for Untt 4, 4-744A and 4-7448). If there is a passive failure which prevents injection through this normal pathe then normally closed valves 3-863A, 3-863B and 3-872 can be opeaed to allow flow to one of the cold legs.

Furhhermore, in the hot leg recirculation mage, which is achieved about 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> after the LOCA, valves 3-863A and 3-863B must be opened.

The single failure that we have identified is the spurious opening of 45Ãhr valve 3-863A or 3-8638 during the injection mode.

This could divert fIow,.

through these valves to either the Refueling Water Storage Tank ot /he due/ion og /he RHR pumps.

In either event. it is nest Hear that suC'ficient water is entering the vessel.

A possible way of correcting this apparent problem is by looking out power to valves 3-863A and 3-863B (For Unit 4, 4-863A and 4-863B).

We We have brought this situation to the licensee,'s attention and he ageees that we have a valid concern.

The response to our guestion 421.2 is acceptable in that FPL has committed that the FPL gA program, applicable to field operations affected by the replacement of the steam generators, will be that described in FPL's gA Topical Report, FPLTgAR 1-76A. Rev.

2 except as amplified in Section 3.6.1 of Rev. 3 of the Steam Generator Repair Report.

The response to our guestion 421.3 is acceptable except that the response on page A-13-1 of Rev. 3, dated April 1978, should be modified as follows:

The statement "FPL and Bechtel comply with the intent of Regulatory Guide 1.38...." should be changed to read:

Under Active Review The Reactor Safety Branch has completed a preliminary review of Flor-ida Power and Light Company's Steam Generator Repair Report.

Based on this review. we have concluded that we do not have any con-cerns which will affect the design of the new steam generators.

It is expected that we will request some additional information regarding the analysis of potential transients and accidents..

Clarification of some of the information presented in FPL's submittal may also be required.

These concerns may affect operating limits and repair procedures but should not affect the steam generator design, cc:

As the first order of'usiness, FPL scheduled the inspection of the Unit 3 steam generators.'nit 3

had operated since January 1977 vrithout any measureable primary to secondary leakage through the steam generators.

At present, 5.7% of the tubes in the Unit 3 steam generators are plugged.

Based on the preliminary inspection results, FPL estimates that they may plug an additional 5$ of. the tubes to operate through cycle 5.

FPL expects to be ready to startup Unit 3 by January 22, 1977.

NRC approval is required before Unit 3 can resume power operation.

FPL estimates that the results of the steam generator inspections and evaluation will be submitted to us by January 3, 1978.

'n fiovember 1 and 2, I attended the training program for an identification badge at Turkey Point Station.

I attended the training with D. Solberg.

RES and J. Lee, ETSB and,the INEL people who will be performing the Inplant

'easurement Program at Turkey Point.

On November 1, 1977, we were given a half day of videotape and live instructor training in radiation protection and the emergency plan.

Me were also given the "Turkey Point Plant Health Physics Training Manual" to study<

Later. whole body counts were performed on us and urine samples were taken from,us.

On November 2, 1977, we took a test to prove that we had gained adequate knowledge of Turkey Point's radiation p'rotection procedures.

Then the others were given "red" identification badges allowing them.unescorted access to the 4Radiation Control Area (RCA)."

However, due to my lack of experience as a "hands on" radiation worker, I was given a "blue" badge.

I will have to be escorted at all times inside the "RCA."

The health physicist, L. Cushen, said that I could get a "red" badge if I went through their week-long training for ",first time" radiation workers.

Since Cushen assui ed me that the INEL people would escort me inside the "RCA," I decided that further training and the "red" badge were not necessary for me.

TURKEY POINT UNITS 3 5 4 lte learned on 7/1/77 that, after the completion of refueling and the inspection of steam generators, FPL discovered that 11 row one tube plugs were missing.

These plugs are about 6 inches in length and 0.775 inch in diameter.

FPL is in process of opening up the reactor again to try to find these missing plugs.

This will delay the re-start of Unit 4, and the schedule is not determined.

Potentially this incident may become a big problem should they not be able to find @11 these plugs, because they do not have a loose part monitoring system on site.

Besides, what assurance do we have that the plugs would not come loose and get back to the reactor again?

This 'makes the reliability of the-explosive plugging method question-able.

Ile are.following'the development closely.

FPL may request an extension of the Unit, 3 operation if they cannot, et Unit 4 back to power before the scheduled shutdown for Unit 3 was scheduled to come down on 7/16/77).

FPL reported the leak rate was approximately 0.3 gpm prior to shutdown Unit No. 4 will remain shutdown for approximately 8 weeks.,

During the shutdown period FPL will refuel the reactor and perform an extensive inspection of the steam generators at Unit No, 4.

Donald.M. Elliott, Project Manager, Operating Reactors Branch 83, DOR George Lear, Chief, Operating Reactors Branch P3, DOR STEAM GENERATOR TUBE LEAK AT TURKEY POINT UNIT NO. 4 On January 25, 1977, Turkey Point Unit No. 4 was shutdown to repair a'9 gallon per hour tube leak in steam generator 4C.

The leaky tube was determined to be in Row 45 Column 51.

This is the second Unit No. 4 shutdown resulting from a steam generator tube leak since December 3, 1976.

Since the leaky tube is adjacent to the previously plugged tube in Row 45 Column 50 (Nemo for K. R. Goller, from D. M. Elliott, January 25, 1977),

FPL also plugged the tube in Row 45 Column 49 in a ef'fort to prevent recurring leaks in this area.

The tube leak was determined by FPL to be located at or just below the bottom support plate.

Following the plugging operation for the above two tubes and during the secondary system pressure

check, a slight leak was detected in the RoQ 22 Column 56 tube.

FPL also plugged this tube.

The leak location within the tube was not determined.

The tube leaks were not associated with small bend radius tubes.

Unit No. 4 was returned to power, following tube plugging, on January 30, 1977.

The licensee proposes to use data telemetered from the South Dade meteorological tower in lieu of using the station tower.

He request that the Hydrology-Meteorology Branch review the data submitted and determine whether the South Dade information is adequately representa-tive of the Turkey Point site from the standpoint of determining the im-pact of routine effluents and accidental releases and whether the proposed telemetry of the data is acceptable.

Several years of onsite meteorological data from the South Dade site shou'Id'e availab'le.

Provide joint frequency distributions (in the. format described in Regulatory Guide 1.23) by atmospheric stability (defined by the vertical temperature gradient between the ll.G m and 57.9 m levels) for the South Dade site for at least a tw>>year period of record.

Data recovery for the entire period should be greater than 90$.

Hourly averaged data may also be provided on magnetic tape using the enclosed format.

oy letter of Nay 15, 1978, Florida Power and Light Company submitted additional information concerning the proposed decommissioning of the present Turkey Point meteorological measurements program.

This additional information does not affect the meteorological assessment for the Turkey Point Appegdix I evaluation.

DOR TURKEY POINT 3 - STEAN GENERATOR TUBE LEAK Qn July 19, 1978 a small leak was detected, fn Turkey Point Unit 3 Steam Generator C.

Monitoring the leak all day 7-20-78 to determine the magnitude, it was found to range from 0.106 gph to 0.177 gph.

(Technical Specifications limit permits up to 0.3 gpm or 18 gph).

Monitoring of the leak is continuing.

Present plans are to bring Unit'2 (fossil) back on line as soon as possible, shut down Unit 3 for repafr and hold Unit 4 at SOS power.

This fs expected to delay Unit 4 shutdo)In for refuelfng and steam generator inspection by approximately one week from the scheduled date of August 1, 1978.
