The sequence of events was as follows:

(1)

At 9:30 p.m., 4/15/77, the 4A and 4B BAT pumps failed to start for normal makeup.

Concentrated boric acid supply was available to Unit 4 via the 3A and 3B BAT pumps.

(2)

At 3:30 a.m., 4/16/77, the 4A BAT pump was test run satisfactorily after preliminary investigation and returned to operable status.

The 4B BAT was isolated for major maintenance.

(3)

At 2:30 a.m., 4/17/77, the 4A BAT pump tripped on thermal overload and was declared inoperable.

(4)

The 4B BAT pump was rebuilt and returned to operable status at 2:30 p.m., 4/17/77.

CAUSE DESCRIPTION (Continued) equalizing line (used for pump thrust balance) becoming blocked by boric acid crystals.

,A design change for installation of a different type pump is currently under review.
