BREACH OF CONTAINMENT INTEGRITY AIRLOCK EQUALIZING VALVE OPEN A.
INTRODUCTION Our Abnormal Occurrence report 251-75-9 of June 20,
- 1975, reported a breach of containment integrity.
A flow path from containment to atmosphere had been inadvertently established through the equalizing valve for the personnel access airlock outer door.
The position of the valve operating handle indicated that the valve was shut,
- however, the mechanical linkage which connects the operating handle to the valve was not connected.
The linkage is hidden from view behind a metal plate.
B.
CORRECTIVE ACTION The proposed permanent corrective action was to add a check point to the "blind flange list" requiring verification that the mechanical linkages for the airlock inner and outer door equalizing valves are connected.
Subsequent analysis has revealed that a more comprehensive procedural change would provide better assurance of containment integrity.
Therefore, rather than add a single checkpoint to the "blind flange list", we will revise Turkey Point Units No.
3 and 4 Operating 8200 HELPING BUILD FLORIDA
