Request for Amendment of License No. 08-03309-01 Date: Wednesday, August 01, 2018 2:50:18 PM License No.: 08-03309-01 Docket No: 030-01323 Control No: 609400 Licensee Name: Children's National Medical Center This refers to your request to amend your license dated July 2, 2018, In order to continue our review of your request, the following additional information is needed:
Section 8.11, "Radiation Safety Officer (RSO)" of NUREG 1556 Vol. 9 Rev 2 discusses training and experience requirements for Radiation Safety Officers.
Your July 2, 2018, letter did not include the documented structural educational program and preceptor attestation as required by 10 CFR 35.50.
Please complete a NRC Form 313A (RSO) and submit it documenting these items.
Your reply must be an originally signed and dated letter.
The letter may be scanned and submitted as a pdf document attached to an email; or it may be transmitted by facsimile to (610) 337-5269; or it may be sent by regular mail.
