609427 03030105 06-28068-01 Mr. Bedoya, This refers to your request to amend your license dated July 12, 2018.
In order to continue our review of your request, the following additional information is needed:
- 1. Please confirm in writing who the new Senior Principal/Office Manager for your office and include updated contact information (i.e. phone number, fax number).
- 2. In your amendment request you stated that you will be asking to remove the 655 Winding Brook Drive, Glastonbury, CT. address from your license after you complete your move of your portable gauges to 77 Krieger Lane, Suite 802, Glastonbury, CT.
Please note that the corporate address listed on your license is 655 Winding Brook Drive, Glastonbury, CT. and that if you are moving your corporate address that you must state in writing what address you would like listed on your license.
- 3. Please include a diagram of the new storage location that includes: approximate location of the portable gauges and the uses of space surrounding the proposed storage area.
- 4. Please note all correspondence with the NRC should be signed by a person that can make binding legal/financial decisions for your company or provide in writing, a document/letter, that you as the RSO has the authority to make binding legal/financial decisions for your company which is signed by a person that can make binding legal/financial decisions for your company.
