Operating License DPR-58 Docket Ho. 50-315 AEO No. 50-315/76-03 While preparing for steam generator moisture carryover testing, it was determined that during previous testing of this nature on April 6, 7-and 8, 1976, the steam generator blowdown radiation monitor (R-19) was placed in the "reset" position.

This allowed blowdown to continue with excessive levels of tea" '.

This resulted in a violation of Technical Specifications, Appendix 8, Paragraph 2.4.2g which requires blowdown treatrznt if R-19 (steam generator blowdown radiation monitor) is not in service.

Investigation revealed that R-19 was put in the "reset" position so that blowdown would continue.

This>>as mistakenly done without recognizing the technical specification requiring the operation of R-19.

Sampling of the blowdown was conducted during and following the testing.

This sampling indicated the total t/a'ctivity released was

.308 curie, with the highest release rate being

.114e of the technical specification limit for topaz".

This release rate did not present a hazard to the health and safety of the public.

To prevent recurrence, test and staff personnel are now aware of the technical specification requiret-.. nt that blowdown treatment is required if R-19 is not in s. rvice.
