During a" recent inspection by Mr. K. R. Baker, Resident Inspector, it was noted that the monthly functional test of the containment atmosphere gaseous monitoring, system R-11 was not completely performed on.January 7,
l975.
At the start of the
- test, another RMS channel was malfunctioning which caused a
standing "High Radiation" alarm.
During ihe performance of the surveillance test, this alarm could not be functionally tested because of the condition described above.
Retesting when the faulty channel was repaired was overlooked until this require-ment was noted by Mr. Baker.
On February 6, 1975, the "High Radiation" annunciator was checked through partial performance of surveillance test procedure
"~THP 4030 STP.052.
The annun-ciator operated satisfactorily.
To avoid recurrence of this non-conformance, plant instruc-tions have been revised and now require prompt (within 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br />) reporting of an inability to perform a required surveillance test in its entirety on its scheduled due date.
This report is in the form of a surveillance testing status report (Attachment No.
1 to PMI 4030 Revision 1).
Full compliance has now been achieved.
