Dear Ms. Kassel,
A review of your current license indicates that a list of Authorized Users and their associated Client Site was last submitted in 2011.
Additionally, a list of coaches and the clients serviced was last submitted in 2011.
Finally, the delegation of authority to the RSO to sign amendment requests on behalf of the licensee was last submitted in 2011 for the prior RSO.
Please resubmit:
- 1. A current list of Authorized Users and their associated Client Site.
- 2. A list of coaches and the current clients serviced by each coach.
- 3. Confirmation that these lists will be provided to the NRC via notification annually.
- 4. A new delegation of authority to you, signed by senior management, which includes your authority to sign license amendments on behalf of Alliance.
In addition, please confirm:
- 1. that the receipt of licensed material is only to coaches and secured as described in your license renewal;
- 2. that the 525 S. Gould Street, Owasso, MI site is only used for storage of Sr/Rb waste; and
- 3. that the list of Authorized Users is current.
