Reynolds Memorial Hospital, Inc.License No. 47-08019-01 Docket No. 030-03384 Mail Control No. 609024 To support removal of the requested location of use, please provide the following:
- 1. Provide the date of last use of licensed material at 426 8 th Street. In addition, the survey indicates a survey of 1 stress room. The letter dated 3-1-15 indicates that there were 2 stress rooms. Please confirm that both stress rooms were surveyed.2. Please confirm that all radioactive waste was disposed of by decay-in-storage.
- 3. Please submit the current inventory maintained at 800 Wheeling Avenue that includes the sources transferred from 426 8 th Street.4. Please submit the last inventory and leak test of sealed sources performed at 426 8 th Street prior to the transfer of the sources. The NRC's inspection on 10-8-15 indicates that 5 sources were maintained at this facility.5. Please indicate whether Dr. Dharawat should be removed from the license with the removal of this facility. In addition, Item D of the letters referenced in License Condition 14 will be removed since the material described therein was removed from the license.
