SUITE 210 LISLE, IL 60532-4352 JUL 2 6 2018 Enclosed is Amendment No. 6 to your U.S. Nuclear Regulatory Commission (NRC) Material License No. 24-32149-01 for Cardiovascular Medicine in accordance with your requests dated May 5 and June 13, 2018. Your letters are available electronically from the NRC's Agencywide Documents Access and Management System (ADAMS) at Accession Nos. ML 18128A350 and ML 18165A126, respectively.
Please review the enclosed document carefully and be sure that you understand all conditions.
If there are any errors or questions, please notify the U.S. Nuclear Regulatory Commission, Region Ill office at (630) 829-9887 so that we may provide appropriate corrections and answers. In your letter dated June 13, 2018, you provided the history of use and close-out survey and wipe test results for Cardiovascular Medicine located at 1531 West 32nd Street, Joplin. Missouri.
You also confirmed that all residual licensed material used at this location had been removed -such that any remaining residual radioactivity is within the limits of Title 1 O of the Code of Federal Regulations (CFR) Part 20, Subpart E. Based on its review of close-out survey information, the staff has concluded that all licensable radioactive material has been removed from the location of use and residual radioactive material attributable to licensed activities in this location of use does not exceed current NRC criteria.
Based on these conclusions no further remediation or actions with respect to NRC regulated material is required.
Your facility at 1531 West 32nd Street, Joplin, Missouri is suitable for unrestricted use and NRC license No. 24-32149-01 is hereby terminated.
