Dear Mr. Mullady:
This refers to the letter dated April 24, 2018, requesting, in part, a change in the Radiation Safety Officer (RSO) for NRC License No. 24-32462-01 MD to Kevin F. Grant, R.Ph. However, insufficient supporting information was provided for Mr. Grant. Please provide a statement delineating his duties and responsibilities as RSO (reference NUREG 1556 Vol. 13, Rev. 1, section 8.7.1 and Appendix H. Please also submit an RSO Delegation of Authority signed by Mr. Grant and a senior manager, an example of which can be found in a different volume of NU REG 1556, in Volume 9, Revision 2, Appendix I, last page. Both are on our website at http://www.nrc.gov or just "google" the titles provided for quick access to each. Within 15 da s of the date of this letter (by July 18, 2018 , please provide only one complete, writ en response to t ese issues a 1s curren y e and signed by a senior management.
