In your July 3, 2018 response, you state that the decommissioned afterloader was removed from Sanford Health by Zsolt Balog, Varian Brachy Service Engineer on June 14, 2018. Please submit documentation of the disposition of the old HOR unit, as appropriate, showing it has been transferred properly and is no longer at Sanford Health as stated in your July 3, 2018 response to my request for additional information so we can document removal of the old HOR unit from your license, accordingly.
