Clarifications to amendment request dated March 21, 2018 I spoke with M
- r. Perez regarding the amendment to add the use of I0125 seeds for treatment of ocular tumors. 1. What survey instrument is available for use during treatments
? - A model 14c with an end
-window GM will be used.
- 2. If they are doing 6 patients per day, how will they determine correct patient and correct plaque? They over estimated the number they expect to treat. Realistically they will only treat 3 patients per month to start. Patient identity will be checked using 2 forms of ID IAW the WD.
- 3. ISOAID provides a treatment activity based upon the WD and request by the facility. How do they confirm activity and plan? The AMP or therapist will verify and confirm the activity based upon the decay sheet provided by ISOAID. Furthermore, all treatment plans will be double checked within 3 days of administration.
- 4. ISOAID has a shielded patch, will you use one? Yes, they order a protector cover from ISOAID for use after implantation.
- 5. I asked him about the Model CDC
-T1 source listed under 35.400. I stated this was for a cs
-137 source and asked him they ever possessed any. He stated "No".
