Items subject to this Fisher Information Notice (FIN) are confined to those serial numbers and customer orders referred to in the table below. Specifically, affected items are studs, nuts, and cap screws used for securing the Vee-Ball Valve Body to an actuator on only the below listed Customer Purchase Orders. Other orders for Vee-Ball valve body assemblies are not affected by this FIN.

Purpose:
The purpose of this FIN is to alert affected customers that, as of 07 May 2018, Fisher became aware of a situation which may affect the performance of the aforementioned equipment, including its safety-related function. Fisher is informing affected customers of this circumstance in accordance with Section 21.21 (b) of 10 CFR 21.
Applicability:
This FIN applies only to the subject equipment supplied by Fisher as identified above.
Discussion:
Upon review of an order for replacement parts of a valve assembly, an individual was reviewing the parts originally selected for dedication and drew into question the processing level of supplied mounting parts, specifically the bolting used to secure a Vee-Ball valve body to an actuator. An investigation was opened to determine if the bolting had been provided in the previous order and if the bolting should have been classified as a basic component.

An investigation was opened to examine all safety-related rotary valve orders between 1990 and the present to identify any other orders that may have had an identical issue. The investigation revealed a total of (2) customer orders contained body-to-actuator mounting bolting that had not been selected for commercial grade dedication. All other orders of similar construction had bolting correctly selected for commercial grade dedication.
Action Required:
Fisher will work with affected customers to replace any affected parts discussed within this FIN.
